Support Plan
- We will identify your needs though the Needs Assessment and Support Planning process
- We will involve you and incorporate your views into the support plan
- We will ask you to sign the Support Plan to ensure that you agree with it
- We will provide you with a copy of your Support Plan if you want one
- We will provide you with a leaflet explaining what the Support Plan is about
- We will ask you how you would like to receive the support either in your home, in the housing
office, or by telephone
Dealing with your support needs
- We will keep in regular contact with you - at least once every four weeks
- We will deal with agencies on your behalf and advocate where necessary
- We will get agencies to assist and support you
- We will endeavour to deal with your support needs
- We will monitor your claim for Housing Benefit on a weekly basis
- We will work closely with the rents, antisocial behaviour, repairs or tenancy management team
in dealing with your issues if necessary
- We will write letters on your behalf
